Conditions

1. Husband and wife must be working with combined income income not less than RM36,000.00 p.a.

2. Must have children under 12 or alternatively parents who needs special attention (Original letter certified from doctor).

Document required for Application

Kindly provide and enclosed the following documents in order that your application can be processed without any untoward delay:

Photocopies

1. NRIC of Husband
   
2. NRIC of Wife
   
3. Marriage Certificate
   
4. Birth Certificates of all children (if pregnancy, original letter from doctor)
   
5. Residence water, electrical or telephone bill (if address different from I/C)

Note:- Two sets of these items will be required.

Originals

1. Passport sized color photos of husband & wife*
   
2. a. Company letter of husband & wife draft in Bahasa Malaysia (original) and 3 months pay slip of husband & wife (under employment)
   
   b. If self employed, to provide:
   - ORIGINAL company letter in Bahasa Malaysia
   - Company registration form (Borang A & D / Form 24 & 49)
   - *Latest 3 month pay slip
   
3. Previous year or current year Form B / BE from LHDN and proof of payment of Tax due (Husband and wife)
   - Those without proof of payment shall provide ORIGINAL copy of EPF statements.
   
4. Immigration Application Forms – Our Office will provide. (Customers to sign)
